OLD CHICAGO FUND RAISER

 

The Mounds View Baseball Diamond Club is the primary source of funding of our high school baseball program.  The money allocated to the Mounds View High School baseball budget covers four coaches, umpires and a portion of the transportation to games and practices.  Everything else we need for our program must come from an alternative resource. These items include additional coaches, baseballs, helmets, game uniforms, catcher’s equipment, training tools and other baseball related items. The Mounds View Baseball Diamond Club is a necessary tool to ensure that your child has adequate training equipment and professional instruction.

 

For the 2007 season, we will meet our financial goal by selling “Pizza Palz” from Old Chicago Pizza. The Pizza “Palz” sell for $11 each. They are good for any pizza at Old Chicago up to $22 in value. The Diamond Club makes $5 profit from each Pizza Pal sold. You must designate your choice below and return it to your Coach prior to Friday, April 13th.  Your Coach will then turn it over to the Diamond Club.  

 

Please Remember these Important Points………………

 

ü     Each Player is Responsible for Raising $ 100 for the Diamond Club!

ü     Leftover Pizza Pals cannot be returned!

ü     If you don’t return this form, you will be responsible for selling 20 pizza palz!

 

 

INSTRUCTIONS

 

1.                  Old Chicago Pizza Pals sell for $11 each. 

2.                  They are good for a pizza up to $22 in value at ANY Old Chicago location.

3.                  They may not be sold within 500 feet of an Old Chicago restaurant.

4.                  The money raised pays for items for all baseball teams 9th Grade through Varsity.

5.                  Money must be turned in on or before May 9th to your coach, or directly to Coach Downey.  Do not drop money off in the athletic office.

6.                  Tickets cannot be returned! If you fail to sell the tickets, you must purchase them.  These tickets had to be pre-paid and cannot be returned to Old Chicago for a refund.

7.                  Checks should be made payable to the “Mounds View Diamond Club”.

8.         If you chose to contribute $100 in lieu of selling pizza pals, please return your check to ensure proper credit. If you chose to donate $50 and sell 10 pizza pals, please include that money when turning it in to a coach.
